10 Easy Steps to get your RDA License
Step 1 - Print Work Experience Form - you need 1280 hours or 4 months of school + 11 months of work
Click the link above to print the work history form (Needs to be filled out by your Employer) * You need to send this form along with your RDA License Application with copies of the required certificates. (See Below)
step 2 - dental radiation safety certification
Make a copy of your Dental Radiation Certification
Step 3 - Coronal Polishing Certificate
Make a copy of your Coronal Polishing Certificate
step 4 - 8 hour infection control certificate
Make a copy of your Infection Control Certificate
Step 5 - CA DEntal practice act certificate
Make a copy of CA Practice Act Certificate. This course can be taken online. It is only 2 hour course.
Step 6 - CPR - Basic Life Support
Make sure it is Basic Life Support. Make a copy of the Certificate. American Red Cross offers BLS training. Visit www.redcross.org
Step 7 - Print the RDA License APPLICATION - The application includes the "Live Scan" Form
Click above to print the Dental Board application for RDA License. Include all the copies of the certificates along with the Live Scan Form (Fingerprinting). *** The application has *Live Scan Form*. You need to take the Live Scan Form to a Fingerprinting Service near you. The form is to be filled in by the Fingerprinting Agent.
LIVE SCAN (FINGERPRINTING)
Search Live Scan Business near you. Call to make an appointment and go for fingerprinting. Take the Live Scan form which you printed (see step 7)
Step 8 - Submit the application online @ WWW.BREEZE.CA.GOV or by Mail
All the copies of the certificates are required along with the Live Scan Form .

Step 9 - Study for your RDA exam!
While you are waiting to get a packet from the Dental Board of California, I strongly suggest to start studying for your RDA Exam. Purchase the "RDA Written and Law & Ethics Study Guide" from Amazon to studying for your RDA Exam. It is the best resource you can find that makes studying easy for you, covering only the topic areas that you will be tested for.

Step 10 - CALL PSI (TEST CENTER) after YOU RECEIVE A PACKET FROM THE DENTAL BOARD OF CALIFORNIA
It takes about 40 days to get a packet form the CA Dental Board. The Packet from the dental board has a list of PSI Locations for you to choose one nearest to you. Call PSI nearest to you to make an appointment for your RDA Exam. I recommend checking your application status online @ BREEZE.CA.GOV
